General Guidelines

Documentation should be typed on letterhead stationary, be recorded on documentation forms available through the Disability Services office, or be in report form.

Documentation should include the following:

     1. A clearly stated diagnosis of a disability

     2. The signature, printed name, title, and professional credentials of the evaluator (doctor, psychologist, recognized as eligible to diagnose the disability).

     3. His/her recommendations for accommodation (based upon limitations within the college academic environment)

      4. The date of evaluation

      5. Currently prescribed medications.

 

Documentation should be current, within three years of enrollment at LCCC. Exceptions will be considered on an individual basis and circumstances of the disability.

Documentation should be a comprehensive assessment addressing the ability to function in a postsecondary academic environment. An IEP is not sufficient documentation by itself; it may be included to support the psycho-educational evaluation or other medical documentation.

 

Additional Documentation Information

Attention Deficit/Hyperactivity Disorder (ADD/ADHD)
An evaluation by a psychiatrist, neurologist, licensed psychologist, or other qualified professional in the field.

Blind/Low Vision
An evaluation report from an ophthalmologist or optometrist regarding the measurement of visual acuity.

Head Injury/Traumatic Brain Injury
An evaluation or medical report from a physician, neurologist, rehabilitation specialist psychiatrist, or qualified professional in the field.

Hearing Impaired/Deaf
An evaluation or medical report from a physician or audiologist including an audiogram.

Physical and Systemic Disorders
A medical statement or report from a physician, neurologist, psychiatrist, or other medical specialist in the field.

Psychiatric/Psychological Disorders
An evaluation report by a mental health professional, licensed clinical social worker, psychologist, psychiatrist, neurologist that includes the DSM-IV diagnosis.

Specific Learning Disability
A comprehensive psycho-educational evaluation from a clinical or educational psychologist, school psychologist, neuropsychologist, learning disability specialist, medical doctor, or other qualified professional; OR a school district evaluation report with diagnostic data and a list and descriptive analysis of all test and subtest scores, dated within three years of enrollment.
