BASIC REQUIREMENTS FOR ADMISSION TO
LAKE CITY COMMUNITY COLLEGE
Applicant MUST:
- Have graduated with a diploma, or equivalent G.E.D., from an accredited high school.
- Submit a general application to the Office of Admissions at Lake City Community College.
- Take the standardized placement test (ACT, CPT, or SAT). If deficiencies are found in Reading, Math or Writing, the student must complete college prep courses in these specific areas before college courses may be attempted. Test scores must be dated within two years of application. Reading can be exempted by a standard placement test with a score of at least 18 on ACT, 83 on CPT or 440 on SAT.

PHYSICAL THERAPIST ASSISTANT PROGRAM
Applicant MUST:
- Apply to the technical phase (second year) of the PTA program by March 1 of each year. After March 1, we can not guarantee your space in the technical phase (second year) of the PTA program. PTA program is a “selective enrollment program.” Applicants’ records are evaluated by the Program Coordinator and Faculty. Final acceptance in the technical phase (second year) of the program is contingent upon student’s academic record (GPA), completion with a grade of “C” or better of all the prerequisite courses, and satisfactory results from a physical examination, drug screen, and Florida Department of Law Enforcement (FDLE) background check.

Schedule an appointment to meet with the PTA Program Coordinator to apply for the technical phase (second year) of the PTA program. The purpose of the meeting is to file an application to the technical (second year) of the PTA program. Please bring the following items to the meeting: Clinical Experience Verification Form; Summary of Clinical Experience.

Complete all prerequisite courses with a grade of “C” or better by the end of the Spring semester for the year applying.

Have taken the sciences (Anatomy and Physiology I and Anatomy and Physiology II) within the last five years prior to application. Both Anatomy and Physiology courses must have a lab.

Have completed at least 24 hours of clinical exposure in a physical therapy setting. This experience may be gained through observation, volunteer work or employment as an Aide. Clinical experience must be acquired under the auspices of a Licensed Physical Therapist or Licensed Physical Therapist Assistant. The enclosed Clinical Experience Verification form must be signed by the supervising clinician (see Forms). This form must be completed and submitted to the PTA Program Coordinator.

FINANCIAL AID
Financial Aid is available to qualified students through the Lake City Community College Office of Financial Aid (386-754-4283). Second year PTA students may be eligible for Company Sponsored Scholarship Programs that exist within the profession.
EMPLOYMENT
Phase II of the PTA curriculum is academically demanding. The transition from general education courses to a technical/clinical curriculum requires adjustment and a full time commitment. Employment may be possible on a part time basis, but full time employment is discouraged. Employment is strongly discouraged during full time clinical affiliations.
