ADMISSION REQUIREMENTS




REQUIREMENTS FOR ADMISSION (Pharmacy Tech ATD):

THE APPLICANT MUST:

  • be a high school graduate or have a G.E.D.;
  • complete a Lake City Community College application and submit the appropriate application fee;
  • complete the Test for Adult Basic Education (TABE) with a score of Math 11, Language 10, Reading 10;
  • complete a Pharmacy Technology Program Supplemental Application;
  • receive academic advisement from the Pharmacy Technology Program Coordinator.

AFTER ACCEPTANCE:

This is a limited enrollment program After acceptance into the Pharmacy Technician ATD, the students must submit the following items to the program coordinator:

  • documentation of a recent physical, which includes proof of immunization records (Hepatitis, MMR, & Chickenpox and a PPD skin test);
  • proof of  accident and health/hospitalization insurance;
  • proof of a negative drug screen for controlled substances;
  • a ‘Request for FDLE Criminal History Form’ to the program coordinator;
  • proof of four hours of AIDS in Florida continuing education;
  • proof of CPR-Health Care Provider certification.

Students must provide their own transportation to all area/regional clinical facilities.

A student should not assume admission into the program until they are officially notified by the Pharmacy Technology Program Coordinator.

Some coursework is sequential in nature. Students must make a "C" or better in the all Pharmacy (PTN) courses to continue to the next semester of the program.


REQUIREMENTS FOR ADMISSION (Pharmacy Technician Management AAS):

THE APPLICANT MUST:

  • be a high school graduate or have a G.E.D.;
  • complete a Lake City Community College application and submit the appropriate application fee;
  • complete a placement test as prescribed by State Board of Education Rule (CPT, ACT or SAT.) If an applicant does not obtain acceptable scores, preparatory coursework may be required.
  • complete a Pharmacy Technology Program Supplemental Application;
  • receive academic advisement from the Pharmacy Technology Program Coordinator.

AFTER ACCEPTANCE: 

This is a special access program. Students must also be accepted into the Pharmacy Technician ATD to be able to complete the degree requirements. After acceptance into the Pharmacy Technician ATD, the students must submit the following items to the program coordinator:
